Building Connections Through Community Outreach
Helping Hands Hawai‘i recently spent time with the team at Makaha Community Mental Health Center, connecting with case managers and staff to strengthen coordination around shared clients and services.
During the visit, our team met with Case Manager Keona Leslie Jr. and other staff members to talk through the day-to-day realities of supporting clients. Conversations focused on how our programs can better align, especially when it comes to communication, coordination, and ensuring individuals don’t fall through the cracks.
We also had the chance to visit Kauhale Lahilahi (Makaha Clubhouse) and reconnect with a client, highlighting how long-term relationships and trust continue to play an important role in this work.
Moments like these are a reminder that strong partnerships between organizations matter. When providers stay connected, it creates a more consistent and supportive experience for the individuals and families we serve.
Helping Hands Hawai‘i is available to meet with case management teams, providers, and partner organizations for similar outreach visits. These conversations help ensure services are aligned and that support is reaching those who need it most.
